The Medical Treatment Release Form must be completed by all DCHS students/families. The 2025-2026 form is due by Friday, September 5, 2025.

The Medication Administration Release Form is optional and only filled out by DCHS students/families that want prescription and/or OTC medication dispensed during school or outside school on a retreat/field trip.
